1. PSA Birth Certificate
2. High School/Senior High School Report Card (F138)

4. For Hawak Kamay (HK) Scholars (if awarded):

  1.1 Fill up Student Permanent Record (SPR) & Info Sheet

  1.2 Verification of Required Documents (if available):

  Step 2Registrar and Finance

  2.1 Encoding of SPR

  2.2 Admission

  2.3 Submission of Requirement (if available)

  PSA Birth Certificate

  High School/Senior High School Report Card (F138)

  Form 137 (may follow)

  2.4 Downpayment

  College Non-scholars: can pay P1,000.00 for registration fee

  College Scholars: can pay P750.00 for registration fee

  SHS: can pay P500.00 for registration fee

  2.5 Issuance of Official Receipt (OR) of down payment.

  Step 3Marketing Department

  3.1 Verification of issued OR number

  3.2 Printing of ORF (Official Registration Form) or COM (Certificate of Matriculation)

  3.3 Join designated Facebook groups and fill up ID Application Form
